  • Fruit :
    • La naringénine (flavonoïde, flavanone) est anti-arythmique, elle entraine un allongement de l’espace QT [1]
    • Le jus est inhibiteur enzymatique
    • Les furanocoumarines et la naringine (flavonoïde, flavanone) sont des inhibiteurs potentiels de l'activité du CYP3A4
      • L'inhibition du métabolisme intestinal de simvastatine, lovastatine ou atorvastatine par le jus de pamplemousse expose au risque majeur de rhabdomyolyse parfois mortelle par augmentation de leurs concentrations sériques de 300 à 1 600 %. De plus un seul verre de jus de pamplemousse peut inhiber pendant près de 3 jours l'activité CYP3A4 intestinale. En conséquence, l'utilisation de ces trois statines chez les consommateurs de jus de pamplemousse doit être proscrite et la pravastatine, la fluvastatine ou la rosuvastatine (peu ou pas métabolisées par le CYP3A4 intestinal) doivent être recommandées chez ces patients [2]
    • Le jus est hypoglycémiant et hypocholestérolémiant [3]
    • La poudre d'écorce du fruit est hépatoprotectrice vis-à-vis des lésions induites par le tétrachlorure de carbone [4], hypoglycémiante, hypolipidémiante [5], améliore le profil lipidique chez le rat [6]
  • Pépins :
    • L'extrait de pépins de pamplemousse (EPP) est réputé antibactérien (Salmonella sp. (flavanones : naringine, hespéridine) [7], [8], [9] et antifongique, même si les preuves manquent (des conservateurs synthétiques faisant partie de la majorité des produits sur le marché) [10]
    • Activité sur les formes mobiles et kystiques de Borrelia burgdorferi [11]
    • Protection vis-à-vis de la pancréatite induite par mécanisme d'ischémie-reperfusion [12]
  • Feuilles :
    • L’extrait de feuilles est hépatoprotecteur vis-à-vis des lésions induites par le paracétamol [13], antioxydant, anti-hyperglycémiant [14], pourrait jouer un rôle favorable dans la prévention de la stéatose hépatique [15]

Effets indésirables éventuels et précautions d'emploi

  • Attention aux effets d'inhibition des cytochromes P450 (modification du métabolisme des médicaments à faible marge thérapeutique dans le sens d'une augmentation des concentrations plasmatiques), due à une synergie d'action entre flavonoïdes et coumarines
    • Les furocoumarines présentes dans le pamplemousse (surtout dans la pelure) sont des inhibiteurs irréversibles des cytochromes 3A4 (enzyme) intestinales et des PGP (transporteur d'efflux) intestinales. Ainsi, toute molécule (médicaments) substrat de ces enzymes et/ou de ces transporteurs verra sa concentration sanguine augmenter
    • Précisions de l'AFSSAPS [3] : la bergamottine et la 6,7 dihydroxybergamottine, présentes dans le pamplemousse, exercent un effet inhibiteur puissant du CYP3A4, ce qui augmente l'absorption intestinale des médicaments substrats du CYP3A4. Les conséquences sont une majoration de leurs effets indésirables dose-dépendants, équivalant à un surdosage. Les médicaments à rirque sont ceux qui ont un index thérapeutique étroit. Il s'agirait de :
      • la simvastatine, et dans une moindre mesure, l'atorvastatine, la lovastatine
      • les immunosuppresseurs (tacroIimus,ciclosporine ... ), avec risque accru de néphrotoxicité
      • le cisapride, avec un risque de torsades de pointes
  • Attention à la qualité des produits à base de pépins de pamplemousse, la plupart sont frelatés avec des produits chimiques [17], [18], [19], [20], tous les produits testés ayant été contaminés par les benzethonium et benzalkonium chloride (BTC) [21]
  • Interaction avec les cytochromes P450, inhibiteur de l’activité des CYP3A4 et CYP2C9 in vitro [22], [23]
  • Le risque d'interaction ne semble pas concerner l'huile essentielle qui ne contient pas de flavonoïdes ni de coumarines
